How To Get Started
A free initial 10 minute phone consultation is offered to each client to assist in determining the clinical needs of the client. At that time, an appointment can be scheduled. If insurance is being used, please check with your insurance company regarding your "behavioral health or mental health benefits." Contact information can be located on the back of your insurance card. If using insurance, please arrive 10 minutes early for your first appointment in order to complete the required insurance forms.
For all new clients (except those using Medicaid or Fee For Service), please review and complete the Information and Consent for Services form, as well as either the Adult Client Information form or Child Client Information form. To obtain privacy information, please review HIPAA Privacy Notice. Please bring these forms with you when you come for your first session. For Medicaid referrals, please read below.
For clients using Medicaid, including referrals for state funded Fee for Service program (see Child and Adolescent Core Services), please complete the Referral Form and submit either by sending an email to newclient@valuelives.com, or by sending a fax to 877-505-5278.
